Introduction

In today’s complex healthcare environment, nursing informatics has emerged as a critical component in improving patient care and achieving positive health outcomes (McGonigle & Mastrian, 2017). Nursing informatics focuses on the integration of information technology and data management into nursing practice, enhancing communication, supporting clinical decision-making, and ensuring patient safety. This project proposal aims to introduce a sophisticated Clinical Decision Support System (CDSS) powered by artificial intelligence (AI) within our healthcare organization to revolutionize patient care processes.

Project Description

The proposed CDSS will serve as an advanced platform that leverages AI and machine learning algorithms to analyze patient data and provide real-time decision support to healthcare providers (Wang, Kung & Byrd, 2018). For instance, when a nurse inputs a patient’s vital signs into the system, the CDSS can rapidly analyze this data against evidence-based guidelines and alert the nurse to potential risks or recommend appropriate interventions. Moreover, the CDSS will incorporate predictive modeling capabilities to anticipate outcomes based on historical data and current patient risk factors.

Stakeholders Impacted

The implementation of the CDSS will have a profound impact on various stakeholders within our healthcare organization. Patients will benefit from personalized care plans and timely interventions, leading to improved health outcomes and increased satisfaction (Glassman, 2017). Nurses and healthcare providers will experience enhanced decision-making support, reduced cognitive workload, and streamlined workflow efficiency. Administrators will observe improvements in quality metrics, reduced costs associated with preventable complications, and optimized resource allocation. The IT department will play a crucial role in system integration and maintenance, while the Quality Improvement Team will utilize CDSS-generated data for performance monitoring and quality enhancement initiatives (Thew, 2016).

Patient Outcomes and Care Efficiencies

The introduction of the CDSS is expected to yield significant improvements in patient outcomes and care efficiencies. By providing real-time decision support, nurses and providers can make informed decisions promptly, leading to timely interventions and preventive measures. The system’s alerts and reminders will help reduce medication errors and ensure adherence to treatment protocols. Additionally, the CDSS will facilitate seamless communication and collaboration among interdisciplinary teams, promoting coordinated care delivery and patient-centered approaches. Patients will benefit from improved engagement through user-friendly interfaces and mobile applications, empowering them to actively participate in their care management (Glassman, 2017).

Technologies Required

To implement the CDSS effectively, several cutting-edge technologies will be essential. AI and machine learning algorithms will drive data analysis and predictive modeling, providing actionable insights for healthcare providers (Wang, Kung & Byrd, 2018). Integration with Electronic Health Record (EHR) systems will enable seamless access to patient data, supporting real-time decision-making and care coordination. Patient-centered mobile applications will enhance patient engagement and enable remote monitoring, improving accessibility and convenience. Scalable and secure cloud infrastructure will facilitate data storage, processing, and accessibility, ensuring compliance with privacy and security regulations.

Project Team and Nurse Informaticist Role

The success of this project hinges on the collaboration and expertise of a multidisciplinary team. A project manager will oversee planning, coordination, and stakeholder engagement. The Nurse Informaticist will play a pivotal role in leading the design, implementation, and optimization of the CDSS, ensuring alignment with nursing workflows and standards (Sipes, 2016). Data analysts and IT specialists will handle data integration, system development, and technical implementation. Clinical champions will engage frontline staff to promote adoption and usability of the CDSS. The Quality Improvement Team will leverage CDSS-generated data for continuous quality improvement initiatives and outcome measurement (Macieria et al., 2017).

Conclusion

In conclusion, the implementation of a sophisticated Clinical Decision Support System driven by nursing informatics will revolutionize patient care within our healthcare organization. By harnessing the power of AI and technology, we aim to enhance patient outcomes, improve care efficiencies, and empower healthcare providers to deliver high-quality, evidence-based care.

Part 1: Global Health Comparison Matrix

Aspect United States Japan
Health Issue Aging Population Aging Population
National Policies The United States addresses the challenges of an aging population through a combination of federal programs such as Medicare, Medicaid, and the Older Americans Act. Medicare provides health insurance for Americans aged 65 and older, covering hospitalization, outpatient care, and prescription drugs. Medicaid supports low-income elderly individuals, covering long-term care services. The Older Americans Act funds community-based services like nutrition programs and caregiver support. Japan’s response to its aging population is anchored in its Long-term Care Insurance System (LTCI) and the Medical Insurance System for the Aged. The LTCI, introduced in 2000, provides universal coverage for long-term care services, including home care and institutional care. The Medical Insurance System for the Aged ensures healthcare access for elderly citizens through insurance coverage for medical services and medications.
Strengths of Policies The strength of U.S. policies lies in their comprehensive coverage for seniors. Medicare emphasizes preventive care, reducing the burden of chronic diseases. The Older Americans Act supports community-based programs, promoting social engagement and well-being among the elderly. Japan’s policies excel in ensuring universal coverage for long-term care, reducing financial barriers to essential services. The focus on community-based care fosters independence and reduces institutionalization rates among the elderly.
Weaknesses of Policies Despite its comprehensive coverage, the U.S. healthcare system faces challenges related to high costs and disparities in access. The cost of Medicare and Medicaid continues to rise, impacting government budgets. Disparities in healthcare access persist based on socioeconomic factors, affecting the quality of care for vulnerable elderly populations. Japan’s aging population poses funding challenges to its healthcare system. The LTCI system faces financial strain due to the growing number of elderly beneficiaries. Access to services remains a challenge in rural areas, where healthcare infrastructure is less developed.
Social Determinants of Health In the United States, social determinants such as income, education, and geographic location significantly impact the health outcomes of the elderly. Low-income seniors face barriers to accessing quality care, resulting in health disparities across populations. Geographic disparities in healthcare access contribute to variations in health outcomes among elderly individuals. In Japan, social determinants like strong social support networks and cultural attitudes towards aging influence the health and well-being of the elderly. Family-based care remains prevalent, with cultural norms emphasizing filial piety and intergenerational support.
Government’s Approach (Cost, Quality, Access) Cost: The U.S. healthcare system faces escalating costs driven by technological advancements and an aging population.

Quality: Emphasis on innovation and advanced technologies leads to high-quality care but also contributes to rising healthcare expenditures.

Access: Disparities persist in healthcare access, impacting the quality of care received by elderly individuals.

Cost: Japan’s healthcare system grapples with the financial burden of caring for its aging population.

Quality: The emphasis on community-based care and integrated services ensures high-quality care for the elderly.

Access: Challenges in rural areas necessitate investment in healthcare infrastructure to improve access to services for elderly residents.

Impact on Global Health The U.S. influences global health policies through initiatives aimed at addressing aging-related challenges. Research funded by U.S. institutions contributes to global understanding of geriatric care and age-related diseases. Collaboration with international partners enhances knowledge sharing and capacity building in aging-related research and policy development. Japan’s innovative approaches to elder care, particularly its LTCI system, serve as a model for other countries facing aging populations. International collaborations in research and policy development promote the adoption of best practices in geriatric care worldwide.
Impact on Nursing Role The aging population in the U.S. underscores the importance of geriatric nursing. Advanced practice nurses play a critical role in managing complex health issues among elderly patients, emphasizing preventive care and health promotion. Japan’s emphasis on community-based care and interdisciplinary collaboration expands the role of nurses in gerontological care. Geriatric nurses in Japan are integral to care teams, providing holistic support to elderly patients and their families.
Impact on Local Healthcare Organizations In the U.S., healthcare organizations are adapting to the needs of an aging population by expanding geriatric services and integrating technology into elder care. Innovations in home-based care and telemedicine improve access and quality of care for elderly patients. Japan’s healthcare organizations prioritize community-based services and invest in elder-friendly infrastructure. The integration of technology, such as robotics and telehealth, enhances care delivery and supports aging in place initiatives.

Part 2: Plan for Social Change

As a nurse leader committed to advancing global perspectives in local healthcare practices, advocating for age-friendly policies and fostering international collaborations are essential components of promoting social change. This plan outlines specific strategies to incorporate a global lens into local practice, emphasizing the importance of cultural competence, research collaboration, community engagement, and policy advocacy.

Firstly, advocating for age-friendly policies requires engagement with policymakers and stakeholders to prioritize investments in long-term care infrastructure and community-based services for the elderly. Research has shown that age-friendly environments contribute to improved quality of life and health outcomes for older adults (World Health Organization, 2007). By advocating for policies that support aging in place and promote intergenerational programs, nurse leaders can influence the development of inclusive healthcare systems that address the diverse needs of elderly populations.

Secondly, education plays a crucial role in enhancing cultural competence among healthcare providers. Training programs on cultural sensitivity and diversity are essential to understanding the unique aging experiences of diverse populations (Jeffreys, 2015). By providing ongoing education and resources, nurse leaders can empower healthcare teams to deliver person-centered care that respects individual beliefs, values, and preferences.

Thirdly, fostering international collaborations in aging-related research is vital for identifying best practices and innovative approaches to elder care. Research partnerships enable knowledge exchange and capacity building across borders, leading to improved care outcomes and policy development (Mukaetova-Ladinska et al., 2019). By engaging in collaborative research projects, nurse leaders contribute to the global body of knowledge on aging and promote evidence-based practices in local healthcare settings.

Lastly, community engagement is essential for raising awareness about aging issues and promoting age-friendly initiatives. Community forums, outreach programs, and partnerships with local organizations enable nurse leaders to connect with stakeholders and mobilize resources to support elderly populations (Winters et al., 2020). By collaborating with community leaders and advocates, nurse leaders can drive grassroots efforts that promote social inclusion and well-being among older adults.

Incorporating a global perspective into local practice represents a commitment to advancing health equity and social change. By advocating for age-friendly policies, fostering cultural competence, engaging in collaborative research, and promoting community engagement, nurse leaders can contribute to creating inclusive healthcare systems that prioritize the needs of aging populations both locally and globally.

Introduction

Falls and incidents represent significant risks within nursing homes, demanding careful attention to resident safety by healthcare professionals (Harris, 2016). This evaluation critically examines the effectiveness of fall prevention strategies and incident/accident reporting policies implemented at Crestpark Nursing Home, LLC, with the primary goal of reducing fall-related injuries among residents.

Healthcare Program/Policy Outcomes

The key objectives of the fall prevention and incident/accident reporting policies are explicit: to prevent falls and mitigate associated injuries among residents. Despite the inherent risk of falls in long-term care settings due to age-related factors, the approach involves a comprehensive assessment of each fall incident, including detailed root cause analysis and the development of targeted preventive measures in collaboration with interdisciplinary teams (Milstead & Short, 2019).

Success of the Healthcare Program/Policy

The success of the program is assessed through structured Quality Improvement (QI) meetings involving various healthcare professionals within the facility (Lewinski & Simmons, 2018). These meetings analyze individual case studies and aggregate trends, allowing for the identification of areas for improvement within the policies. Success is defined by a significant reduction or stabilization in fall rates, ultimately leading to enhanced resident safety and well-being.

Number of People Reached

Approximately 300 individuals, including residents, staff members, and family representatives, actively participated in the evaluation process (Harris, 2016). This inclusive approach ensures that policy adjustments are evidence-based and reflective of the diverse perspectives and needs of stakeholders, ultimately enhancing the overall quality of resident care.

Timing of the Evaluation

Regular quarterly evaluations and QI meetings are a routine practice at Crestpark Nursing Home, LLC (Milstead & Short, 2019). Additionally, ad hoc assessments are conducted to address emerging concerns or unexpected trends. The March 2021 evaluation session marked the first meeting in nearly 7 months due to disruptions caused by the COVID-19 pandemic, highlighting the commitment to continuous improvement despite external challenges.

Data Sources

Primary data sources utilized during the evaluation included incident/accident reports, resident medical charts, hospital records, and diagnostic imaging reports (Harris, 2016). These comprehensive data sources facilitated a holistic understanding of each fall incident, enabling informed decision-making and targeted policy adjustments.

Potential Benefits and Unintended Consequences

An immediate benefit of the policies was the prompt notification of falls to designated personnel, ensuring timely assessment and intervention for affected residents (Glasgow et al., 2003). However, the implementation of new reporting protocols may have inadvertently caused feelings of inadequacy among experienced nursing staff. This unintended consequence was addressed through collaborative team discussions and additional training initiatives.

Stakeholders Involved

Stakeholders engaged in the evaluation process included residents, their families, nursing staff, administrative personnel, and the facility’s social worker (Milstead & Short, 2019). The central focus of the evaluation efforts remains resident safety and well-being, with stakeholders playing a crucial role in shaping policy outcomes and improvement strategies.

Evaluation of Success

The evaluation outcomes demonstrate tangible success, evidenced by a decline in fall occurrences and notable improvements in resident mobility and independence (Shiramizu et al., 2016). Ongoing policy assessments and adjustments based on evaluation findings will further strengthen safety protocols and ensure sustained positive outcomes for residents.

Recommendation for Continuation

Continuation of the existing fall prevention and incident/accident reporting policies is strongly advocated due to their demonstrable impact on resident safety and the reduction of fall-related incidents (Williams & Anderson, 2018). Policy refinements will be informed by continuous evaluations, stakeholder feedback, and evolving best practices in long-term care.

Involvement in Future Evaluations

Future involvement in policy evaluations will focus on expanding stakeholder engagement, particularly by soliciting feedback from residents’ families and spending additional time on the floor to gain firsthand insights into policy implementation challenges and successes (Lewinski & Simmons, 2018). This proactive approach will enhance the ability to identify and address emerging issues promptly.

NURS 6050/5050 PA004 Advocating for the Nursing Role in Program Design and Implementation Paper Example

In recent decades, the healthcare landscape in the United States has evolved significantly from a biomedical model towards a biopsychosocial framework, particularly in addressing chronic diseases (Aggar et al., 2021). This shift has seen the emergence of interventions that emphasize social elements of care, such as health coaching, social prescribing, and community-based initiatives aimed at improving overall well-being and disease management (Carnes et al., 2017). Social prescription programs, like music on prescription, have shown promise in enhancing self-care skills, reducing complications, and addressing health disparities by fostering social support networks (Alliance for Healthier Communities, n.d.). To explore the nursing role in program design and implementation, I conducted an interview with a nurse executive manager (NE) from TGK Hospital, discussing the institution’s music on prescription initiative.

Interviewer: Tell us about a healthcare program within your practice. What are the costs and projected outcomes of this program?

Interviewee: The music on prescription program at TGK Hospital aims to connect elderly patients with chronic illnesses, such as Alzheimer’s dementia, cancer, and depression, to community-based music programs. The estimated annual cost of the program is approximately $45,000, with projected outcomes including improvements in general, mental, and emotional well-being, enhanced quality of life, and reduced levels of anxiety and depression (Husk et al., 2019). Ultimately, these outcomes translate into cost savings by reducing hospitalizations and enabling staff to focus on clinical priorities.

Interviewer: Who is your target population?

Interviewee: Our target population primarily comprises elderly patients diagnosed with chronic illnesses, especially those vulnerable to social isolation and its associated health impacts.

Interviewer: What is the role of the nurse in providing input for the design of this healthcare program? Can you provide examples?

Interviewee: Nurses play a crucial role in program design by identifying patients at risk of social isolation and those who would benefit from social prescription interventions. Their frontline experience and patient interactions inform program design decisions and contribute to the effectiveness of interventions (Pescheny et al., 2018).

Interviewer: What is your role as an advocate for your target population for this healthcare program? Do you have input into design decisions? How else do you impact design?

Interviewee: As a geriatric nurse practitioner, I advocate for vulnerable elderly patients by leveraging my clinical experience to highlight the impact of social isolation on health outcomes. I actively participated in evidence-based research to support the adoption of music on prescription, influencing program design and implementation (Aftab & Shah, 2017).

Interviewer: What is the role of the nurse in healthcare program implementation? How does this role vary between the design and implementation of healthcare programs? Can you provide examples?

Interviewee: Nurses facilitate program implementation by establishing patient referrals and connecting them with appropriate services. During program design, nurses contribute to screening procedures and operational frameworks, ensuring alignment with patient needs. In implementation, nurses act as program champions, fostering community engagement and monitoring outcomes (Pescheny et al., 2018).

Interviewer: Who are the members of a healthcare team that you believe are most needed to implement a program? Can you explain why?

Interviewee: A successful program implementation requires collaboration across disciplines. Physicians are essential for identifying patients eligible for social prescriptions. Nurses, including community-based assistants, bridge patients to services based on individual needs. Hospital management plays a crucial role in resource allocation and strategic support for implementation (Alliance for Healthier Communities, n.d.).

Conclusion

The interview with TGK Hospital’s NE highlights the integral role of nurses in program design and implementation, particularly in social prescription initiatives like music on prescription. Nurses’ expertise in patient care and community engagement ensures that programs are tailored to meet the needs of vulnerable populations, ultimately improving health outcomes and reducing healthcare costs.

Detailed Assessment Instructions for the NURS 6050/5050 PA004 Advocating for the Nursing Role in Program Design and Implementation Paper Assignment

Assignment: Advocating for the Nursing Role in Program Design and Implementation

As their names imply, the honeyguide bird and the honey badger both share an affinity for honey. Honeyguide birds specialize in finding beehives but struggle to access the honey within. Honey badgers are well-equipped to raid beehives but cannot always find them. However, these two honey-loving species have learned to collaborate on an effective means to meet their objectives. The honeyguide bird guides honey badgers to newly discovered hives. Once the honey badger has ransacked the hive, the honey guide bird safely enters to enjoy the leftover honey.

Much like honeyguide birds and honey badgers, nurses and health professionals from other specialty areas can—and should—collaborate to design effective programs. Nurses bring specialties to the table that make them natural partners to professionals with different specialties. When nurses take the requisite leadership in becoming involved throughout the healthcare system, these partnerships can better design and deliver highly effective programs that meet objectives NURS 6050 Policy and Advocacy for Improving Population Health.

In this Assignment, you will practice this type of leadership by advocating for a healthcare program. Equally as important, you will advocate for a collaborative role of the nurse in the design and implementation of this program. To do this, assume you are preparing to be interviewed by a professional organization/publication regarding your thoughts on the role of the nurse in the design and implementation of new healthcare programs.

To Prepare:

  • Review the Resources and reflect on your thinking regarding the role of the nurse in the design and implementation of new healthcare programs.
  • Select a healthcare program within your practice and consider the design and implementation of this program.
  • Reflect on advocacy efforts and the role of the nurse in relation to healthcare program design and implementation.

The Assignment: (2–4 pages)

In a 2- to 4-page paper, create an interview transcript of your responses to the following interview questions:

  • Tell us about a healthcare program, within your practice. What are the costs and projected outcomes of this program?
  • Who is your target population?
  • What is the role of the nurse in providing input for the design of this healthcare program? Can you provide examples?
  • What is your role as an advocate for your target population for this healthcare program? Do you have input into design decisions? How else do you impact design?
  • What is the role of the nurse in healthcare program implementation? How does this role vary between design and implementation of healthcare programs? Can you provide examples?
  • Who are the members of a healthcare team that you believe are most needed to implement a program? Can you explain why?

Introduction

Healthcare policy has been a central focus of recent U.S. presidential administrations, with distinct approaches and priorities shaping the landscape of public health. This paper conducts a comprehensive comparison of the healthcare agendas pursued by Presidents Trump and Obama, highlighting their strategies, resource allocations, and approaches to addressing critical population health concerns. By examining these contrasting approaches, we can glean insights into the complexities of healthcare policymaking and its profound impact on the well-being of the American population.

Part 1: Agenda Comparison Grid

Population Health Concern: Affordable Healthcare Access

Factors Contributing to the Concern:

  • Escalating healthcare costs driven by medical inflation and pharmaceutical expenses (Baker, 2019).
  • Inadequate insurance coverage, particularly among low-income and vulnerable populations (Holahan & Blumberg, 2019).
  • Geographical and socioeconomic disparities leading to unequal access to healthcare services (Williams & Mohammed, 2018).

Administrative Agenda Focus:

President Trump’s Administration:

President Trump’s healthcare agenda centered on the repeal and replacement of the Affordable Care Act (ACA), colloquially known as Obamacare. The administration advocated for market-based reforms, promoting association health plans and short-term health insurance options as alternatives to the ACA. Additionally, there was a significant emphasis on reducing regulatory burdens on healthcare providers, aiming to foster competition and drive down costs (Fiedler, 2018).

President Obama’s Administration:

President Obama’s administration achieved a landmark victory with the passage and implementation of the Affordable Care Act (ACA), which aimed to expand healthcare coverage and improve access to care. The agenda prioritized the expansion of Medicaid coverage for low-income individuals, the regulation of insurance markets to ensure essential health benefits and pre-existing condition coverage, and the promotion of preventive care and population health initiatives to curb escalating healthcare costs (Blumenthal & Collins, 2014).

Allocation of Resources:

President Trump’s Administration:

Under President Trump, resources were directed towards combating the opioid epidemic, with increased funding allocated for addiction treatment and prevention programs. The administration also prioritized expanding telehealth services in rural areas to improve access to healthcare. Notably, there was a reduction in funding for ACA outreach and enrollment programs, reflecting the administration’s stance on shifting away from federal intervention in healthcare markets (Dobson & DaVanzo, 2018).

President Obama’s Administration:

President Obama’s administration allocated significant resources towards enrollment assistance and outreach programs under the ACA to facilitate coverage expansion. Investments were made in community health centers and preventive care initiatives to address disparities in healthcare access and promote population health. Funding was also directed towards mental health services and substance abuse treatment (Sommers & Epstein, 2014).

Approaches to the Issue:

President Trump’s Administration:

President Trump’s approach to healthcare emphasized state flexibility and deregulation, empowering states to design healthcare programs tailored to their specific needs. Market-based reforms were favored over federal mandates, with an emphasis on consumer choice and affordability. The administration sought to dismantle key provisions of the ACA through executive actions and regulatory changes, advocating for a more decentralized and competitive healthcare system (Antos & Capretta, 2017).

President Obama’s Administration:

President Obama’s healthcare approach revolved around federal interventions to expand coverage and regulate insurance markets. The ACA introduced subsidies to make insurance affordable for low-income individuals and established regulations to protect consumers from discriminatory practices by insurers. The administration promoted preventive care and public health initiatives to address underlying health issues and reduce long-term healthcare costs (Rosenbaum, 2011).

Part 2: Agenda Comparison Grid Analysis

Administrative Agency Responsible:

The implementation and oversight of healthcare policies under both administrations were primarily the responsibility of the Centers for Medicare & Medicaid Services (CMS). This agency played a critical role in administering Medicaid expansion, managing healthcare exchanges, and implementing regulatory changes affecting the healthcare industry (Glied & Ma, 2015).

Inclusion on the Presidential Agenda:

For President Trump, the inclusion of healthcare issues on the agenda was influenced by conservative groups advocating for market-based reforms and regulatory rollbacks. Concerns over rising premiums and limited consumer choice also drove the administration’s policy priorities. In contrast, President Obama’s healthcare agenda was propelled by public outcry over escalating healthcare costs, Democratic policy priorities, and the imperative to expand coverage for uninsured Americans (Marmor, 2012).

Champion/Sponsor:

President Trump’s healthcare agenda was championed by Republican lawmakers, business associations, and conservative think tanks advocating for smaller government involvement in healthcare and increased market competition. President Obama’s healthcare initiatives garnered support from progressive advocates, healthcare providers, and consumer organizations pushing for universal coverage and equitable access to care.

Part 3: Narrative

Communication with Policymaker

Dear Honorable [Title] [Legislator’s Name],

I am writing to emphasize the critical importance of ensuring affordable and equitable access to healthcare services for all Americans. As a registered nurse deeply invested in public health, I urge you to prioritize policies that promote comprehensive healthcare reform.

The contrasting approaches of recent presidential administrations, particularly those of Presidents Trump and Obama, underscore the complexities of healthcare policymaking. While the Trump administration prioritized market-based reforms and deregulation, the Obama administration pursued federal interventions to expand coverage and protect consumers.

As we navigate the evolving landscape of healthcare policy, it is imperative to build upon successful initiatives while addressing persistent challenges. Nurses, as frontline healthcare providers, witness firsthand the impact of policy decisions on individuals and families. We advocate for evidence-based solutions that prioritize affordability, accessibility, and quality of care.

I sincerely urge you to consider bipartisan solutions that bridge ideological divides and prioritize the well-being of our communities. By working collaboratively, we can advance policies that ensure affordable and equitable healthcare access for all Americans.

Thank you for your attention to this critical matter.

Sincerely,

[Your Name]

Registered Nurse

Conclusion

In conclusion, comparing the healthcare agendas of Presidents Trump and Obama provides valuable insights into the diverse approaches to addressing critical population health concerns. While President Trump emphasized market-based reforms and deregulation, President Obama pursued federal interventions to expand coverage and protect consumers. Nurses play a pivotal role in advocating for evidence-based policies that prioritize affordability, accessibility, and quality of care. By analyzing these contrasting approaches, we can inform future policy decisions and advance towards a more equitable and sustainable healthcare system.

Healthcare has been a policy item and a topic of debate not only in recent times but as far back as the administration of the second U.S. president, John Adams. In 1798, Adams signed legislation requiring that 20 cents per month of a sailor’s paycheck be set aside for covering their medical bills. This represented the first major piece of U.S. healthcare legislation, and the topic of healthcare has been woven into presidential agendas and political debate ever since.

As a healthcare professional, you may be called upon to provide expertise, guidance and/or opinions on healthcare matters as they are debated for inclusion into new policy. Introduction

The process of agenda setting within U.S. presidential administrations significantly influences healthcare policy formulation, impacting national priorities and strategies. This paper undertakes a detailed comparison of how Presidents Trump, Obama, and Bush approached key healthcare issues during their respective terms in office. By analyzing their agendas and actions, valuable insights into the complexities of healthcare policymaking at the highest levels of government are gained.

Agenda Setting in Presidential Administrations

Agenda setting is a critical process whereby certain issues gain prominence and become top priorities for policymakers. In the federal context, this process is influenced by factors such as public opinion, economic conditions, interest group pressures, and presidential leadership (Kingdon, 2001). Understanding how presidents prioritize healthcare issues provides insights into the policy-making process and its impact on healthcare outcomes.

President Bush’s Initiatives

President George W. Bush’s administration prioritized advancing market-oriented reforms and strengthening healthcare infrastructure. Notably, Bush championed the Medicare Modernization Act of 2003, a significant legislative effort aimed at improving access to prescription drugs for seniors through Medicare Part D (Milstead & Short, 2019). This initiative introduced market-based competition among private insurers, aiming to enhance affordability and access to medications for Medicare beneficiaries.

Furthermore, President Bush promoted Health Savings Accounts (HSAs) as a mechanism to empower individuals to manage their healthcare costs and make informed healthcare decisions (Milstead & Short, 2019). HSAs offered tax advantages and flexibility in paying for medical expenses, aligning with Bush’s emphasis on consumer-driven healthcare solutions and individual responsibility.

Additionally, Bush’s administration prioritized strengthening public health preparedness, particularly in response to emerging infectious diseases and bioterrorism threats (Milstead & Short, 2019). This proactive approach underscored the importance of enhancing healthcare infrastructure and responsiveness to public health emergencies during his presidency.

President Obama’s Legacy

President Barack Obama’s administration achieved a monumental milestone with the passage of the Affordable Care Act (ACA) in 2010. The ACA aimed to expand health insurance coverage, improve access to healthcare services, and regulate the health insurance industry (Milstead & Short, 2019). Key provisions included the establishment of health insurance exchanges, Medicaid expansion, and protections against insurance discrimination based on pre-existing conditions.

President Obama’s healthcare agenda prioritized achieving universal healthcare coverage and reducing disparities in access to care (Milstead & Short, 2019). The ACA significantly reduced the uninsured rate and enhanced healthcare quality through value-based care initiatives and accountable care organizations.

President Trump’s Reform Efforts

President Donald Trump’s administration pursued a different path, focusing on reforming healthcare policies established under the ACA. The Trump administration advocated for the repeal and replacement of the ACA, emphasizing reducing federal involvement in healthcare and promoting market-driven solutions (Milstead & Short, 2019). Efforts to dismantle key aspects of the ACA, such as the individual mandate and Medicaid expansion, were pursued through legislative initiatives like the American Health Care Act (AHCA).

Additionally, President Trump prioritized addressing the rising costs of prescription drugs, proposing measures to enhance price transparency and competition in the pharmaceutical industry (Lamb et al., 2015). Furthermore, combating the opioid crisis was a key agenda item, with initiatives aimed at expanding access to addiction treatment (O’Rourke et al., 2017).

Comparison and Alternative Strategies

Comparing the approaches of Presidents Trump, Obama, and Bush underscores the diverse strategies employed to address healthcare challenges. President Bush’s market-oriented reforms emphasized consumer empowerment through HSAs and private sector competition. President Obama’s legacy, embodied by the ACA, focused on expanding coverage and regulating the insurance industry to improve access and quality of care. President Trump’s administration prioritized deregulation and market-driven reforms, seeking to reduce federal involvement and enhance individual responsibility.

Alternative strategies could involve a hybrid approach, combining elements of market competition with regulatory safeguards to ensure equitable access and affordability. Emphasizing preventive care, addressing social determinants of health, and leveraging technological innovations could enhance healthcare efficiency and effectiveness.

Conclusion

Analyzing how Presidents Trump, Obama, and Bush handled healthcare challenges provides valuable insights into the complexities of healthcare policy-making and the diverse strategies employed to address national priorities. President Bush’s administration emphasized market-oriented reforms, including initiatives like Medicare Part D and Health Savings Accounts, to promote consumer empowerment and enhance healthcare infrastructure. President Obama’s legacy was marked by the passage of the Affordable Care Act (ACA), aiming for universal healthcare coverage and improved access through regulatory measures and expanded insurance options. Conversely, President Trump pursued deregulation and market-driven reforms, advocating for the ACA’s repeal while targeting prescription drug costs and the opioid crisis. Comparing these approaches underscores the varied strategies used to tackle healthcare issues. Future policy discussions should explore hybrid models combining market competition with regulatory safeguards to ensure equitable access and affordability, prioritizing preventive care, addressing social determinants of health, and leveraging technological innovations to enhance healthcare efficiency and effectiveness nationally.

Speaker Notes:

  • Welcome to today’s staff development presentation on the regulation of nursing practice. Today, we will explore the roles of boards of nursing (BoNs) and professional nurse associations, focusing on their impact on nurse practitioners and healthcare delivery.

Slide 2: Boards of Nursing vs. Professional Nurse Associations

Title: Understanding the Differences

  • Boards of Nursing (BoNs):
    • State agencies regulating nursing practice (National Council State Board of Nursing, Inc., 2022).
    • Ensure public health and safety through licensure and oversight (Short & Loversidge, 2019).
  • Professional Nurse Associations:
    • Private organizations advocating for nursing profession (American Nurses Association, 2021).
    • Focus on professional development and advocacy

Speaker Notes:

  • BoNs oversee licensure and ensure nurses meet competency standards, whereas professional nurse associations promote professional development and advocate for nursing interests.
  • Specifically, BoNs are state agencies responsible for regulating nursing practice to protect public health and safety. They oversee the licensure and discipline of nurses, ensuring compliance with state laws and standards.
  • In contrast, professional nurse associations are private organizations that advocate for nurses’ interests and professional development.

Slide 3: Membership and Composition of State Boards of Nursing

Title: Composition of BoN

  • State-specific board compositions (e.g., Florida Board of Nursing) (Florida Board of Nursing, 2022).
  • Appointment process by Governor and confirmation by Senate (Toney-Butler & Martin, 2021).

Speaker Notes:

  • Each state’s BoN comprises appointed members representing various stakeholders. For example, the Florida Board of Nursing (FBON) includes nurse practitioners, registered nurses, non-nursing professionals, and laypersons. Members are typically appointed by the Governor and confirmed by the Senate.

Slide 4: State Regulation Impacting General Nurse Scope of Practice

Title: General Nurse Scope of Practice

  • Overview of state regulations (e.g., Florida Nurse Practice Act) (Florida Board of Nursing, 2022).
  • Influence on nurse roles, responsibilities, and standards of care

Speaker Notes:

  • State regulations define permissible duties and responsibilities for registered nurses, ensuring standardized and quality care delivery.
  • In Florida, the Nurse Practice Act outlines the scope of practice for registered nurses. This regulation influences nurses’ roles by defining permissible duties and responsibilities. It impacts healthcare delivery by ensuring standardization and quality of care, which can influence patient outcomes and safety.

Changes in regulations aim to improve healthcare access, quality, and cost-effectiveness. For instance, telehealth provisions during the COVID-19 pandemic have facilitated remote care delivery.

Slide 9: Future Trends and Challenges

Title: Looking Ahead

  • Anticipated Future Trends in Nursing Regulation
    • Expansion of Scope for Advanced Practice Nurses: There is a growing trend towards expanding the scope of practice for advanced practice registered nurses (APRNs), allowing them to practice more independently and take on greater responsibilities (American Nurses Association, 2021).
    • Increased Emphasis on Telehealth: Telehealth is expected to continue evolving, with more states enacting policies to support and regulate telehealth services. This trend is driven by the need for accessible and convenient healthcare delivery, especially in rural and underserved areas (Park & Yu, 2018).
    • Enhanced Focus on Interprofessional Collaboration: Nursing regulation is likely to emphasize interprofessional collaboration to improve patient outcomes and healthcare delivery. This includes collaborative practice agreements and team-based care models (Park & Yu, 2018).
  • Challenges and Opportunities for Nurses and Healthcare Systems
    • Regulatory Compliance and Adapting to Change: Nurses and healthcare systems will face challenges in adapting to evolving regulations, requiring continuous education and training to stay compliant (Short & Loversidge, 2019).
    • Addressing Healthcare Disparities: Nursing regulation will need to address healthcare disparities, focusing on equitable access to care and culturally competent practices (Courtemanche et al., 2018).
    • Leveraging Technology for Quality Care: Opportunities exist to leverage technology for improved patient care outcomes, including electronic health records, remote monitoring, and artificial intelligence applications (Park & Yu, 2018).

Speaker Notes:

  • The future of nursing regulation is shaped by various emerging trends and challenges. Firstly, there is a movement towards expanding the scope of practice for APRNs, allowing them greater autonomy in delivering care. Additionally, telehealth is poised to become more prevalent, especially as policies supporting telehealth services continue to evolve across states. Interprofessional collaboration will also play a key role in future nursing regulation, emphasizing teamwork and coordinated care.
  • In terms of challenges and opportunities, nurses and healthcare systems will need to navigate regulatory compliance and adapt to changing requirements. Addressing healthcare disparities and leveraging technology for quality care delivery will also be important focuses. By embracing these trends and addressing challenges, nurses can contribute to improving healthcare access and outcomes in the coming years.

Detailed Assessment Instructions for the NURS 6050/5050 PA003 Regulation for Nursing Practice Staff Development Meeting Presentation Assignment

Assignment: Regulation for Nursing Practice Staff Development Meeting

Nursing is a very highly regulated profession. There are over 100 boards of nursing and national nursing associations throughout the United States and its territories. Their existence helps regulate, inform, and promote the nursing profession. With such numbers, it can be difficult to distinguish between BONs and nursing associations, and overwhelming to consider various benefits and options offered by each.

Both boards of nursing and national nursing associations have significant impacts on the nurse practitioner profession and scope of practice. Understanding these differences helps lend credence to your expertise as a professional. In this Assignment, you will practice the application of such expertise by communicating a comparison of boards of nursing and professional nurse associations. You will also share an analysis of your state board of nursing.

To Prepare:

  • Assume that you are leading a staff development meeting on regulation for nursing practice at your healthcare organization or agency.
  • Review the NCSBN and ANA websites to prepare for your presentation.

The Assignment: (8- to 9-slide PowerPoint presentation)

Develop a 8- to 9-slide PowerPoint Presentation that addresses the following:

  • Describe the differences between a board of nursing and a professional nurse association.
  • Describe the board for your specific region/area.
    • Who is on the board?
    • How does one become a member of the board?
  • Describe at least one state regulation related to general nurse scope of practice.
    • How does this regulation influence the nurse’s role?
    • How does this regulation influence delivery, cost, and access to healthcare?
  • Describe at least one state regulation related to Advanced Practice Registered Nurses (APRNs).
    • How does this regulation influence the nurse’s role?
    • How does this regulation influence delivery, cost, and access to healthcare?
    • Has there been any change to the regulation within the past 5 years? Explain.
  • Include Speaker Notes on Each Slide (except on the title page and reference page)

Introduction

In the dynamic landscape of healthcare policy, legislative actions shape the future of healthcare delivery and access in the United States. This paper critically analyzes two proposed bills from the 116th Congress, highlighting their legislative intent, target populations, current status, and implications. The bills under scrutiny are H.R.1390 – Mobile Health Record Act of 2019 and H.R.1346 – Medicare Buy-In and Health Care Stabilization Act of 2019.

Part 1: Legislation Comparison Grid

Health-related Bill Name: H.R.1390 – Mobile Health Record Act of 2019

Description:

The bill aims to amend the Social Security Act to allow Medicare and Medicaid beneficiaries access to their personal medical data, including prescriptions, providers, diagnoses, and test results, through mobile health record applications.

Federal or State: Federal

Legislative Intent:

The legislative intent of H.R.1390 is to enhance patient access to healthcare information by leveraging mobile technology. By allowing beneficiaries to access their medical data conveniently, the bill seeks to empower individuals to make informed decisions about their healthcare.

Target Population:

The target population includes beneficiaries of Medicare and Medicaid.

Status of the Bill:

Introduced on February 27, 2019, H.R.1390 is currently under review by the Committee on Energy and Commerce and the House Committee on Ways and Means.

Proponents/Opponents:

Proponents of the bill advocate for improved patient autonomy and healthcare transparency. Opponents may have concerns about data security and the efficacy of mobile health records.

Press Coverage:

Given its recent introduction, H.R.1390 has not yet garnered significant press coverage.

General Notes/Comments:

The bill addresses a critical need for enhanced patient engagement and information access in healthcare, potentially revolutionizing how individuals interact with their medical data.

Health-related Bill Name: H.R.1346 – Medicare Buy-In and Health Care Stabilization Act of 2019

Description:

H.R.1346 proposes amendments to Medicare to allow individuals aged 50 to 64 to buy into the program, addressing gaps in coverage for dental, vision, and hearing care.

Federal or State: Federal

Legislative Intent:

The intent is to expand Medicare eligibility to a younger age group and improve coverage for essential health services not currently included in traditional Medicare plans.

Target Population:

Vulnerable populations, specifically individuals aged 50 to 64, who face challenges in accessing affordable healthcare coverage.

Status of the Bill:

Introduced on February 25, 2019, H.R.1346 is currently under review by the Committee on Ways and Means and the Committee on Energy and Commerce.

Proponents/Opponents:

Proponents support expanded healthcare coverage and reducing out-of-pocket costs for older adults. Opponents may argue about the financial implications of expanding Medicare eligibility.

Press Coverage:

Similar to H.R.1390, H.R.1346 has not received extensive press coverage due to its recent introduction.

General Notes/Comments:

The bill seeks to address critical gaps in Medicare coverage, particularly for preventive services like dental, vision, and hearing care, which are essential for maintaining overall health and well-being.

Part 2: Legislation Testimony/Advocacy Statement

Advocacy Statement for H.R.1390 – Mobile Health Record Act of 2019:

As a healthcare professional, I strongly support H.R.1390 as it promotes patient-centered care by granting individuals access to their medical information via mobile platforms. This bill empowers patients, fosters informed decision-making, and strengthens the patient-provider relationship. Mobile health records have the potential to enhance care coordination and patient outcomes while respecting privacy and data security.

Addressing Opponents:

Opponents of H.R.1390 may raise concerns about data privacy and the effectiveness of mobile health technology. To address these concerns, the bill should include robust provisions for data encryption, secure transmission, and stringent privacy regulations. Additionally, ongoing evaluation and feedback mechanisms should be implemented to ensure the efficacy and scalability of mobile health records.

Recommended Amendment:

To strengthen H.R.1390, I propose an amendment that mandates interoperability standards among mobile health record applications. This amendment would facilitate seamless data exchange across different platforms, enhancing accessibility and usability for patients and providers alike.

Advocacy Statement for H.R.1346 – Medicare Buy-In and Health Care Stabilization Act of 2019:

The Medicare Buy-In and Health Care Stabilization Act of 2019 represents a crucial step towards improving healthcare access for vulnerable populations. By allowing individuals aged 50 to 64 to buy into Medicare and expanding coverage to include dental, vision, and hearing care, this bill addresses significant gaps in our healthcare system.

Addressing Opponents:

Opponents may express concerns about the financial implications of expanding Medicare eligibility. However, evidence suggests that investing in preventive care can yield long-term cost savings by reducing the burden of untreated conditions and complications.

Recommended Amendment:

To enhance H.R.1346, I recommend an amendment that extends coverage to individuals older than 64 who may also struggle with dental, vision, and hearing care costs. This amendment would ensure equitable access to essential health services for all Medicare beneficiaries.

Conclusion

In conclusion, the proposed bills, H.R.1390 and H.R.1346, demonstrate legislative efforts to advance healthcare access and address critical gaps in coverage and information access. Advocating for these bills involves engaging with stakeholders, addressing concerns, and proposing meaningful amendments to strengthen their impact. By supporting these legislative initiatives, healthcare professionals can contribute to shaping policies that promote equitable, patient-centered care in the United States.

Purpose

The extended stress experienced by nurses due to burnout can lead to emotional and physical exhaustion, posing significant challenges within the healthcare sector. Nursing burnout negatively impacts both nurses and patients, resulting in reduced job satisfaction, lower quality patient care, higher rates of medical errors, and increased turnover rates. Contributing factors to nursing burnout include excessive work hours, inadequate staffing, lack of support, and exposure to traumatic events.

It’s crucial to address untreated burnout promptly as it can diminish nurses’ effectiveness and contribute to errors, ultimately affecting patient care quality (Galischa & Sutrimo, 2022). Many studies highlight the necessity of a well-planned policy to combat burnout, proposing various interventions. Healthcare organizations must implement strategies aimed at preventing and managing nursing burnout to support nurses’ well-being and ensure high-quality patient care.

Revised PICO(T)

Among qualified hospital nurses (P), does the implementation of a stress-releasing program (I) compared to no intervention or stress management (C) reduce nursing burnout and increase job satisfaction (O) within a six-month timeframe (T)?

A proper healthcare plan based on credible and recent evidence is essential for designing a policy to intervene in nursing burnout and improve patient care outcomes. This plan addresses critical issues such as staffing, nursing leadership, and overall job satisfaction, emphasizing the importance of taking proactive measures to address this pressing issue.

Population Affected by the Policy

The target population for the intervention to address nursing burnout comprises registered nurses (RNs) employed in acute care hospital settings, particularly those working in high-stress areas like critical care units, emergency departments, and medical-surgical units. Nurses in Intensive Care Units (ICUs) face particularly high stress due to factors such as high mortality rates, severe illness, and ethical dilemmas (Ramírez-Elvira et al., 2021).

This intervention also focuses on RNs who endure extended work hours, high patient-to-nurse ratios, and insufficient support and acknowledgment from their supervisors. It excludes other healthcare professionals like nursing assistants or licensed practical nurses, whose roles and responsibilities differ within the healthcare system.

The scope of this intervention is to provide tailored strategies and support to this specific group of RNs to mitigate the risk of nursing burnout and enhance their well-being and the quality of patient care they provide.

Definitions Section

In this project, it’s important to clarify some complex terms to aid understanding among individuals outside the healthcare field. Here are explanations of key terms used in nursing burnout prevention guidelines:

  • Registered Nurse (RN): An RN is a healthcare professional who has completed a nursing program and passed the National Council Licensure Examination (NCLEX) to obtain a license for nursing practice.
  • Burnout: Burnout refers to a state of physical, emotional, and psychological exhaustion caused by prolonged and intense stress. It occurs when individuals feel overwhelmed, emotionally drained, and unable to achieve their goals (Smith et al., 2023). In the context of nursing, burnout can result from insufficient staffing levels and excessive workload.
  • Interprofessional Collaboration: This term describes the practice of healthcare professionals, such as physicians, social workers, and pharmacists, working together to deliver comprehensive and coordinated care to patients (Peltonen et al., 2019).
  • Maslach Burnout Inventory (MBI): The MBI is a tool used to measure burnout rates among healthcare professionals. It assesses three dimensions of burnout: emotional exhaustion, depersonalization, and personal accomplishment (Williamson et al., 2018).
  • Nursing Stress Scale (NSS): The NSS is a psychological assessment tool designed to measure stress levels specifically among nurses. It is commonly used in research to study the impact of nursing-related stress on nurses’ physical and mental health, job satisfaction, and causes of stress (Bautista et al., 2019).
  • Job Satisfaction: Job satisfaction refers to an individual’s overall positive or negative feelings toward their job or occupation. It reflects the extent to which employees believe that their job meets their expectations, needs, and desires (BasuMallick, 2021).

Policy Statement

This policy aims to combat nurse burnout within our medical center through a comprehensive quality improvement plan. It mandates that all staff nurses adhere to evidence-based guidelines and practices outlined in the policy. The medical center’s administration is responsible for providing the necessary resources to support the plan’s objectives. The policy incorporates strategic measures such as stress reduction techniques, counseling services, fostering compassion, and implementing mentorship programs for nurses.

The organization anticipates a significant reduction in stress among the target population following the policy’s implementation. Specifically, the policy aims to alleviate stress and burnout among nurses by addressing workload issues and ensuring adequate staffing levels to enhance job satisfaction and improve patient care quality. Implementing this policy at the right time is critical to maintaining overall healthcare quality and ensuring the success and sustainability of the plan (O’Donoghue et al., 2021).

Instructions for Policy Implementation – Step-by-Step

Below are the detailed steps for implementing a policy within six months to address nursing burnout among registered nurses in a healthcare setting:

  • Identify Factors Contributing to Burnout: Engage nurses in identifying key factors contributing to burnout. Utilize the Maslach Burnout Inventory (MBI) tool to evaluate emotional exhaustion, depersonalization, and personal accomplishment (Williamson et al., 2018).
  • Assess Stress Levels: Implement a mobile-based electronic survey using the Nursing Stress Scale (NSS) to assess stress levels among nurses (Hendy et al., 2020). Ensure nurses feel comfortable providing honest responses without organizational pressure.
  • Optimize Staffing and Workload: Address burnout by recruiting additional nursing staff to maintain appropriate patient-nurse ratios. This initiative aims to reduce workload, stress, and job dissatisfaction among nurses (Opoku et al., 2022).
  • Provide Follow-up Counseling: After nurses encounter challenging situations, offer follow-up counseling to support their well-being and reduce burnout (Vincent-Höper et al., 2020). Allocate resources for additional personnel to conduct these counseling sessions as needed.
  • Implement Mentorship Programs: Develop mentorship programs to support both new and experienced nurses. Investing in leadership roles for nurses through mentoring can enhance job satisfaction and retention rates (Ephraim, 2021).
  • Foster Compassionate Work Environment: Promote empathy and compassion within the organization to alleviate stress among nurses (Ren et al., 2020). Valuing nurses’ contributions can lead to improved patient outcomes.

Conclusion

Nursing burnout presents a significant challenge in healthcare settings, impacting both nurse well-being and patient care quality. Contributing factors such as inadequate staffing, excessive workloads, and lack of support necessitate a comprehensive approach to addressing burnout. This includes identifying these contributing factors and utilizing validated tools like the Maslach Burnout Inventory (MBI) and Nursing Stress Scale (NSS) to assess stress levels. Implementing evidence-based interventions such as optimizing staffing levels, providing follow-up counseling, and fostering a compassionate work environment are essential components of this approach.

To sustain positive outcomes and drive long-term improvements, it is imperative to implement effective policies and procedures tailored to the specific needs of registered nurses (RNs) in high-stress environments. These initiatives are crucial for enhancing nurse well-being, improving job satisfaction, and ultimately elevating the quality of patient care within healthcare settings. Ongoing evaluation and adaptation of these interventions are critical to ensure continued success in combating nursing burnout and promoting a supportive work environment for healthcare professionals. By prioritizing evidence-based strategies and supportive measures, healthcare organizations can proactively address nursing burnout and enhance overall healthcare outcomes.

NURS FPX 8030 Methods and Measurement Nursing Paper Example

Introduction

Nursing burnout is a prevalent issue among nurses, often resulting from heavy workloads, inadequate staffing levels, and workplace stress. To evaluate and address this problem effectively, tools such as the Maslach Burnout Inventory and the Nursing Stress Scale are commonly used. These tools help assess the effectiveness of interventions aimed at mitigating burnout and improving nursing practice. Additionally, implementing the LEAN model can facilitate changes in nursing practice to reduce burnout and enhance overall processes and outcomes.

Instruments for Evaluation of Implementations Plan

To resolve issues of stress and burnout among nurses, it’s crucial for Directors of Clinical Operations and Nursing Excellence in healthcare settings to assess the suitability of proposed interventions. This assessment can be achieved using appropriate evaluation tools or scales.

The Maslach Burnout Inventory (MBI) stands out as a gold standard and widely used tool for measuring burnout among healthcare professionals, including nurses. Developed by Christina Maslach and Susan E. Jackson, the MBI comprises three subscales: emotional exhaustion, depersonalization, and personal accomplishment. Through the MBI, one can evaluate the effectiveness of interventions by observing changes in scores across these subscales. Its reliability has been demonstrated through extensive research spanning more than twenty-five years (Williamson et al., 2018).

Gray-Toft and Anderson developed the Nursing Stress Scale (NSS) in 1981 to assess stress levels experienced by nurses in their work environment. Composed of scores divided into various stress states, the NSS can identify sources and levels of stress among nurses, aiding in evaluating intervention effectiveness (Hendy et al., 2020).

NSS is appropriate for use in evaluating interventions for nursing burnout due to its specificity for nursing stress, addressing issues directly relevant to nurses’ work environments (Alharbi & Hasan, 2019).

Construct Analysis

The MBI assesses burnout across emotional exhaustion, depersonalization, and personal accomplishment dimensions. Emotional exhaustion refers to feelings of being drained and exhausted from work, depersonalization involves a negative or distant attitude towards patients, and personal accomplishment relates to feelings of competence and achievement in one’s work. This tool employs a 7-point Likert scale for responses, providing comprehensive insight into burnout levels (Williamson et al., 2018; Lin et al., 2022).

The NSS is a self-report questionnaire consisting of 34 items that evaluate stressors inherent in the nursing role, relationships with other healthcare professionals, work environment factors, and coping resources. It employs a 5-point Likert scale for responses, allowing for a detailed assessment of stress levels among nurses (Hendy et al., 2020).

Validity and Reliability

Both the MBI and NSS have undergone rigorous validation through statistical analyses, including factor analysis, Cronbach’s alpha coefficient, and test-retest reliability (Mert et al., 2020; Todorovic et al., 2021). These analyses ensure the consistency, stability, and validity of these measures. They have been extensively used in research and practice globally, with translations available in multiple languages to facilitate their use in diverse healthcare settings.

Credible Resources and Their Outcomes

In-depth research into the application of the Maslach Burnout Inventory (MBI) and Nursing Stress Scale (NSS) has highlighted several valuable studies that can assist nurses in utilizing these assessment tools effectively. Various keywords were employed to access relevant literature from academic databases such as CINAHL, PubMed, and Cochrane.

Erwan et al. (2020) conducted a case study to analyze burnout syndrome among hospital nurses using the Maslach Burnout Inventory – Human Service Survey (MBI-HSS). The study involved 100 nurses from three public hospitals who completed the questionnaire. The findings revealed that most nurses experienced elevated emotional exhaustion, a significant indicator of burnout. Surprisingly, fewer nurses reported high levels of depersonalization, suggesting they did not feel distant or cynical towards patients. Moreover, there was a notable lack of professional fulfillment among these nurses, particularly those under 20 years old.

Another study by Hendy et al. (2020) examined predictive factors affecting stress among nurses caring for COVID-19 patients in isolation hospitals in Egypt. The authors identified various demographic and work-related factors contributing to stress during the pandemic. The study, based on the Nursing Stress Scale (NSS), revealed that insufficient training, work burden, and reduced assistance were the top stressors. Notably, work-related issues such as inadequate resources, lack of personal protective equipment, and insufficient training on infection prevention and control significantly predicted stress levels among nurses.

The Rationale for Credible Resources

The Maslach Burnout Inventory (MBI) and Nursing Stress Scale (NSS) were selected for this study due to their extensive validation and widespread use in addressing nursing burnout. When seeking to apply the MBI and NSS in studies similar to our PICOT analysis on nursing burnout, nurses should conduct comprehensive literature reviews. Peer-reviewed journals like PubMed, CINAHL, and Nursing Open, along with hospital guidelines, are valuable resources for locating related studies. Additionally, exploring other assessment tools such as the Copenhagen Burnout Inventory (CBI), Oldenburg Burnout Inventory (OLBI), Nurse Stress Index (NSI), Perceived Stress Scale (PSS), and Professional Quality of Life Scale (ProQOL) can provide a broader understanding. Combining two or more tools can enhance the assessment of burnout interventions. A recent study comparing the CBI and OLBI with the MBI among doctors found that the CBI is an effective alternative to the MBI (Ogunsuji et al., 2022).

The rationale behind consulting such informative and credible literature is that it helps nurses utilize different tools, parameters, or their combinations to assess and compare their reliability in addressing burnout and stress among nurses effectively.

Similarities

Several studies, like our project in this course, have utilized one or more assessment instruments to evaluate burnout in nursing and the effectiveness of interventions. Many interventions, such as mentorship, compassion, and counseling, have been employed across studies to address burnout. For instance, Vincent-Höper et al. (2020) implemented follow-up counseling as an intervention immediately after nurses faced patient aggression, while Hofmeyer et al. (2020) emphasized compassion and sympathy towards nurses to address burnout. Furthermore, similar statistical analyses have been used to analyze and interpret data collected from these instruments.

Differences

Various studies have employed different instruments and parameters to assess nursing burnout, including the Perceived Stress Scale (PSS), Copenhagen Burnout Inventory (CBI), or Professional Quality of Life Scale (ProQOL). For instance, Tan et al. (2020) used the Oldenburg Burnout Inventory (OLBI), Hospital Anxiety and Depression Scale (HADS), and Safety Attitudes Questionnaire (SAQ) to examine burnout and related factors among different healthcare workers. Differences in populations, demographics, and sample sizes across studies may impact the generalizability of findings. Some studies focus on specific aspects of burnout, such as emotional exhaustion, depersonalization, or reduced personal accomplishment, whereas our project broadly examines burnout and stress in nurses.

Conclusion

In addressing the risk and prevalence of burnout and stress among nurses, employing validated tools like the Maslach Burnout Inventory (MBI) and Nursing Stress Scale (NSS) is essential. These instruments offer valuable insights into nurses’ experiences of burnout and stress, allowing for targeted interventions. However, it is crucial to ensure the validation and suitability of these tools within specific contexts, considering factors such as sample size, demographics, and the quality of parameters utilized.

While existing literature provides a foundation for understanding nursing burnout interventions, further studies are warranted to explore the effectiveness of interventions and the application of diverse assessment instruments. Future research should focus on refining intervention strategies tailored to nurses’ unique stressors and evaluating the comparative effectiveness of different assessment tools in real-world healthcare settings. This ongoing investigation will contribute to the development of evidence-based approaches to mitigate burnout and promote nurses’ well-being.

Detailed Assessment Instructions for the NURS FPX 8030 Methods and Measurement Nursing Paper Assignment

Description

This assessment requires you to put all of your research together in the longest paper in the course, 5-7 pages, including theories and models, the target population, and evaluation tools for assessing the impact of the proposed intervention.
Concepts, theories, and models serve several key functions in health care improvement and help answer the “what, how, and why” for performance improvement. Quality Improvement (QI), concepts, theories, and models help translate research to practice.
Think about a theory or model that will facilitate the implementation of the project. For example, if you anticipate resistance from the stakeholders, or the population undergoing a change, consider aligning a change theory with the model for improvement. Or, if the project focuses on a process change, consider the LEAN model or Donabedian’s Structure, Process, Outcomes model.
Next, focus on identification of the population. Ask yourself, “Who is undergoing the practice change, and who will benefit from the practice change?” Think about how you may want to identify your population. What are the demographics of the population? For example, can they be described by age, generation, socio-cultural determinants, geographic area, gender, or ethnicity? If the population affected includes the staff or providers of care, how might you describe these participants? If the affected population is the patient(s), consider human subject risk when evaluating the literature pertaining to the intervention.
Finally, this assessment also includes how evidence-based instruments are used in health research to generate analyzable data. Sometimes, biometric instruments (such as MRI and laboratory testing), demographic surveys or questionnaires, and psychosocial instruments are used as screening tools to explore relationships between variables of interest. Not all improvement studies require the use of a tool or survey. If you decide to use a tool or survey for the safety initiative, make sure the tool is evidence-based and has been validated for reliability.
